Acknowledgement of Risk and Consent to Participate
I, the undersigned parent or legal guardian of the above-named child, hereby give my full consent for my child to participate in pony riding lessons conducted by Alana Dipasquale.
I understand that horseback riding and being around horses carries inherent risks, including but not limited to: falls, kicks, bites, uneven terrain, unpredictable behavior of animals, and equipment failure. I acknowledge that although every effort will be made to ensure the safety of all participants, accidents can occur.
I further understand that the lessons will take place on property owned by Ashli Blyth and Cameron Blyth, and I consent to my child being present on said property for the duration of the lessons.
Assumption of Risk and Release of Liability
In consideration for my child being allowed to participate in theselessons, I voluntarily assume all risks associated with the activity and agree to release, discharge, and hold harmless:
● Alana Dipasquale (Instructor)
● Ashli Blyth and Cameron Blyth (Property Owners)
● Their family members, employees, volunteers, agents, and representatives
from any and all liability, claims, demands, actions, or causes of action
whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by my child or myself while participating in these lessons or while on the property.
Safety Requirements
I acknowledge the following safety requirements:
● A parent or legal guardian must be present during all lessons.
● It is highly recommended that all participants wear a helmet provided by the parent and appropriate boots while participating in riding activities.
● It is my responsibility to ensure my child is appropriately dressed and equipped for each session.
Medical Authorization
In the event of an emergency, I authorize Alana Dipasquale or the property owners to seek medical treatment for my child as deemed necessary. I agree to assume full financial responsibility for any resulting medical care.
By signing below, I affirm that I have read, understood, and voluntarily agree to the terms above.
