1. Consent to Participate

I, the undersigned parent or legal guardian of the child listed above ("Child"), hereby give my full consent for my Child to participate in the Microschool program conducted at the above private residence. I understand and agree that:

- The Microschool is not a licensed daycare facility. This is not a place of business but rather a home that is being utilized for this learning environment. - The teachers are certified educators with valid First Aid/CPR certifications and fingerprint clearance cards. The home owners as well have their fingerprint clearance and are also CPR certified. Any adults that will be volunteering in the home as a teachers aides will be required to also have fingerprint clearance cards to ensure the safety of the children.

- The learning pod is located on a private property with open acreage and live farm animals (e.g., chickens, goats, dogs, a pony, etc.), which may present certain inherent risks. There is no swimming pool or large water source. However, during the summer months we may utilize a blow up swimming pool with a slide that is no deeper than 1.5ft deep.

- My Child may participate in both indoor and outdoor educational activities, including contact with animals under supervision.

2. Acknowledgement of Risk

I understand that participation in a home-based learning pod and exposure to a farm environment presents potential risks, including but not limited to:

- Allergic reactions or injury due to interaction with animals; - Trips, falls, or other injuries while engaging in outdoor play or exploration;

- Exposure to environmental elements (e.g., dirt, bugs, uneven ground);

- Accidental ingestion or exposure to non-toxic household or farm materials.

I affirm that my Child is physically and mentally capable of participating in such activities.

3. Release and Waiver of Liability

In consideration for allowing my Child to participate in this program, I hereby:

- Waive, release, and discharge the homeowner(s) of the Premises, the certified teachers, and any volunteers or assistants (collectively, the “Released Parties”) from any and all liability, claims, demands, and causes of action of any kind, whether in law or in equity, arising from or related to injury, illness, or damages incurred by my Child or myself in connection with the learning pod.

- Understand this release includes claims arising from the negligence of the Released Parties, but does not include intentional misconduct or gross negligence.

4. Medical Treatment Consent

In the event of an emergency where I cannot be reached, I authorize the teachers or homeowner to obtain necessary medical treatment for my Child, including first aid and emergency transportation. I agree to be responsible for all costs associated with such medical treatment.

5. Indemnification

I agree to indemnify and hold harmless the Released Parties from any and all claims, liabilities, damages, or costs (including reasonable attorney's fees) that may arise from:

- My Child's participation in the “Little Farmers Learning Pod”; - Any act, error, or omission of my Child;

- Any violation of this agreement.

6. Photo/Video Consent

I grant permission for the teachers or homeowner to take photographs or videos of my Child during the “Little Farmers Learning Pod” for the purposes of documentation, internal updates, marketing, social media or sharing with participating families. There will also be live recording for the parents to be able to observe their child while at the learning pod.

7. General Provisions

- This Agreement is governed by the laws of the State of Arizona.

- If any part of this Agreement is found to be unenforceable, the remaining provisions shall remain in full force.

- I have read and understand this entire Agreement and voluntarily sign it.
